In this insightful session, Professor Wael Att, a leading expert in digital implantology, shared his approach to managing full-arch cases with immediacy, offering practical insights for predictable results. The conversation, hosted by Stefan from EAO, explored various strategies for handling complex cases involving failing dentition.

The key to success in full-arch implants, according to Wael, is meticulous planning. He stressed that 90% of a case’s success lies in planning, with only 10% attributed to execution. He emphasized the importance of understanding the patient's needs and discussing multiple treatment options. The three main options range from the simplest (complete dentures) to the most complex (implant-supported fixed rehabilitation). Immediate implant placement and loading, especially in full-arch cases, require careful consideration of bone quality, soft tissue, and the number of implants needed. Typically, six or more implants are used for best results.

Wael also highlighted the significance of digital tools in improving predictability. By using intraoral scans, 3D imaging, and virtual smile design, treatment planning can be optimized, reducing errors and enhancing outcomes. His “Star” concept, which uses three retained teeth to support a surgical guide, was presented as a solution when there are no remaining teeth to anchor guides.

The session also touched on the role of provisional restorations. Wael recommended using machinable materials over traditional metal frameworks due to their superior strength and flexibility. He explained how temporary restorations, when designed and executed properly, can ensure successful long-term results while minimizing the risk of implant failure.

Wael wrapped up by advising on patient care, especially regarding soft diets during the healing phase and ensuring that patients understand the provisional nature of their restorations.

The session proved to be a rich resource for anyone looking to master the predictability of immediate full-arch implant placements, combining advanced technology and proven techniques to achieve optimal results.
